Duplicated logs with Request Workflow for Splunk CoE App?

All,

I have a simple env 10 idexers and a search head no cluster and installed Request Workflow for Splunk CoE App?
https://splunkbase.splunk.com/app/3285/#/details. I am create a user request and ended up dozens of logs of the same content. Seems to duplicate the logs in a distributed env. Does this need to be on a stand alone instance of Splunk? Did I miss something?
