Disk monitoring apps

I want to use the disk monitoring app for monitoring a SMART HDD but i don't find examples for writing code in Inputs conf
Could you help me please???

This is an example of inputs.conf

*Nix platforms

[script://./bin/smartmon_results.sh]

run every hour

interval = 0 * * * *

Please point to that to get all the info you need about all the configuration files needed.
